• Citizen Complaint Form

  Citizen complaints are taken seriously by the City of Hazleton.  Please submit the Citizen Complaint Form and the city will respond in compliance with its policies that the complaint has been received and what findings are discovered about the complaint.

•  Hazleton Employment Application

  Employment applications open positions may be dropped off with Hazleton City Clerk or night drop box.

•  Hazleton Junior Firefighter Application

  The Hazleton Junior Firefighter Program is a training program for young men and women, aged 16 – 18, interested in serving their community and preparing to become actual firefighters upon reaching the age of 18.

• Legion Hall Rental Application

  The American Legion Hall is available for your special occasion or event. Please submit the Legion Hall Rental Application to make your reservation.

• Public Records Examination Request Form

  Public records within the control of the City of Hazleton are available for examination at the City Hall during regular business hours. Please submit the Public Records Examination Request Form, clearly stating the records being requested.

• Utility Service Application

  Submit the Utility Service Application to apply for water, sewer and garage services.
